  1. Residence, curate and lecturer: I reside personally upon my Cure, and in my Parsonage-House: and keep no Curate: and have no Lecturer.

  2. Other benefices: I have but one Benefice.

  3. Divine service at other churches: I perform Divine Service at no other Church besides my own.

  4. Frequency of divine service: Divine Service is performed at my Church twice every Lord's Day; with a Sermon in the Morning.

  5. Frequency of communion: The Sacrament of the Lord's Supper is administered five times a year; & generally between twenty & thirty Communicants at a Time.

  6. Catechising of children: I catechise the Youth of my Parish, between Easter & Michaelmas.

  7. Church fabric: My Church & Chancel are in good Repair - the Church-yard well kept - and all Things decent for Divine Service.

  8. Glebe terrier: I have a Terrier of the Glebe Lands, Houses &c according to the Tenor of the Canon, properly signed.

  9. Number of families: I have about twenty Families in my Parish.

  10. Chapels: I have no Chapel, within my Parish.

  11. Papists, dissenters and meeting houses: There are neither Papists, nor any Dissenters from the established Church, within my Parish.

  12. School, alms-houses, hospital and charitable endowments: There is no Public School, Alms-House, Hospital, nor Charitable Endowment within my Parish: nor parochial Library.

  13. Charity: No Lands, Tenements, or other Benefactions for any pious & charitable uses have been left.
